This is the only health information KIS will hold for your child during Summer School, so please complete every question. It helps us keep your child safe.

Medical Disclosure and Care Acknowledgement

KIS has a school nurse on site, but we may not have staff trained to manage every medical condition or to provide one-on-one support for students with additional needs. To keep all students safe, KIS may decline an application if we cannot provide adequate care. KIS’s decision is final.

You must disclose all medical conditions, disabilities, and special needs on this application. If we later discover that relevant information was not shared, KIS is not responsible for any resulting incident or injury. We may also remove the student from the program right away, with no refund of fees.

AGREEMENT TO SUMMER SCHOOL RULES / REGULATIONS

Welcome to KIS Summer School! These few days are about discovery, creativity, English, and fun in a warm international environment. To keep every child safe and every day running smoothly, we ask each student and parent to read and agree to the rules below before attending. These rules apply to all summer school participants.

1. How we treat each other
  • My child will treat everyone — students, teachers, and staff — with kindness and respect, regardless of background, age, or ability.
  • My child will follow staff instructions the first time they are given, and act honestly.
  • My child will take care of school property and the belongings of others.
  • KIS does not tolerate any bullying or harmful behavior — physical, verbal, social, or online. My child will not hurt, threaten, tease, or exclude others, and will tell a teacher right away if they see it happening.
  • My child will try their best and join in all activities with a positive attitude.
2. English and participation
  • Summer School is an all-English environment. My child will do their best to speak English and join in actively.
  • My child will participate in all activities and follow along with the group.
3. Attendance and arrival
  • My child will attend each day they are signed up for and arrive on time. The building opens at 8:30; the program runs 9:00–15:00.
  • If my child must be absent, arrive late, or leave early, I will notify the KIS office as early as possible with my child’s name and the reason.
  • I understand summer school is a short program, and consistent daily attendance helps my child get the most from it.
4. Lunch and food
  • My child will bring their own lunch from home each day, clearly labeled with their name.
  • For everyone’s safety (including allergies), students may not share food with each other unless a teacher gives permission for a specific activity.
5. Belongings and what to bring
  • I will write my child’s name on everything they bring — lunch, clothing, water bottle, and supplies — so items don’t get lost.
  • My child will wear appropriate clothing and indoor shoes as instructed, and take all belongings home each day.
  • Students should not bring valuables or extra money. KIS is not responsible for lost or stolen money or personal items.
  • Simple appearance only: no cosmetics, jewelry, colored nails, or dyed hair. Long hair should be tied back for active activities and games. (If earrings are worn for cultural or religious reasons, please speak with the office in advance.)
6. Items not allowed at school

To keep everyone safe and focused, my child will not bring:

  • Phones and smartwatches (if a phone is truly needed, it must be handed to staff on arrival and is used only to reach a parent, with staff present)
  • Video game consoles or handheld games (e.g., Nintendo Switch)
  • Trading cards, toys, laser pointers, or personal speakers
  • Knives, sharp objects, or anything that looks like or could be used as a weapon
  • Any other device or item not needed for the day’s activities

Prohibited items will be held by staff and returned to a parent.

7. Health, illness, and injury
  • If my child feels unwell or is hurt, staff will care for them and may call me to come pick them up. KIS cannot give fever or pain medicine.
  • If my child needs to take any medication during the day, I will complete a Request to Administer Medicine form in advance. Staff cannot give medication without it.
  • I will keep my child home if they have a fever or a suspected infectious disease, and inform the office.
8. Permission for emergency medical treatment

I understand that in a medical emergency, KIS staff will first try to contact me at the emergency number I provide. If I cannot be reached, I authorize KIS to seek appropriate medical or surgical treatment for my child, including taking my child to a physician or hospital, and I authorize staff to share my child’s personal and insurance information with medical providers so treatment can begin before I arrive.

9. Permission to use photos / videos

Unless I notify KIS in writing, I give permission for KIS to take photos and videos of my child during Summer School and to use them in KIS marketing materials, including the school website and Instagram (@kiskumamoto).

10. Safe arrival and pick-up
  • I will drive slowly near the school and in the parking lot, and will not let children play in the parking lot.
11. If rules are not followed

To protect the safety and enjoyment of all participants and members of the summer school community, KIS responds to rule-breaking calmly and in clear steps:

  • First time: the student receives a verbal reminder from staff, with a clear explanation of the rule and why it matters.
  • If it continues: KIS contacts the parents to discuss the behavior and agree on how to move forward together.
  • For serious or repeated issues: the student may be removed from the program. In the case of removal for behavior, KIS is not able to offer a refund.

Serious actions — such as violence, bullying, leaving the school grounds without permission, or anything that endangers another child — may move straight to parent contact or removal, regardless of whether a prior warning was given.
